• Single, 16-22 years old
• At least 4th year high school, of good moral character
• at least 5'5" tall

REQUIREMENTS:

• Original and Xerox copy of birth certificate
• Barangay Certification as to residency and endorsement from Barangay Chairman
• Signed Parental Consent
• Certification from school
• One 5R close up picture and one 5R whole body picture
• Medical certificate including drug test
